The most important thing for households to benefit from ACP benefit program is their income. If your household income is at or below 200% of the Federal Poverty Guidelines, or if a member of the household meets at least one of the criteria below:

• Received a Federal Pell Grant during the current award year;
• Meets the eligibility criteria for a participating provider’s existing low-income internet program;
• Participates in one of these assistance programs:
• The National School Lunch Program or the School Breakfast. (Program, including through the USDA Community Eligibility Provision)
• SNAP
• Medicine
• Federal Public Housing Assistance
• Supplemental Security Income (SSI)
• WIC Veterans Pension or Survivor Benefits or Lifeline;
• Participates in one of these assistance programs and lives on

Qualifying Tribal lands:

• Bureau of Indian Affairs General Assistance
• Tribal TANF
• Food Distribution Program on Indian Reservations
• Tribal Head Start (income based)

In order to benefit from the ACP, one of the above criteria must be met. If no household member is eligible for the ACP, it is impossible to benefit.

If you are eligible for this program you must filloutthe ACP Benefit Application Form. This form is also known as FCC Form 5645. The FCC stands for Federal Communications Commissions. 

The beneficiary cannot give his/her benefit to another person, even if they qualify for the ACP. Persons who are eligible for ACP should apply individually on their behalf. Then they can fill in the Acpbenefit.org  Application with their own information.

The second, third and fourth pages of the form contain the applicant’s personal information. On the 2nd page, there are information such as your Name, Surname, Telephone number, Date of birth, e-mail address, authentication method.

You are eligible for the ACP if your income is 200% or less than the Federal Poverty Guidelines. (see the table below). The guideline is based on your household size and state. The table below reflects the income limit by household size. It is 200% of the 2022 Federal Poverty Guidelines.

How to Apply ACP

If you are a Lifeline subscriber, you do not need to fill out the Affordable Connectivity Application Form and apply. How to Apply ACP Online

• Visit the Federal Communications Commission’s official site for an online application for the ACP.
• When you visit the application page of the site, you will see a short form.
• This form consists of name, surname, date of birth, identity verification options.
• After filling in your personal information, choose Social Security Number, Tribal ID Number, Driver’s License, Military ID, Taxpayer Identification Number or other Government ID for authentication.
• At the bottom of the form, “Do you qualify for Lifeline or the Affordable Connectivity Benefit through your child or a dependent?” has a question.
• Tick the appropriate option for this question.
• After answering this question, click the NEXT button.
• Fill in the required information completely and accurately in the next steps.

How to Apply ACP by Mail

The second method of applying to the program is by mail. Print out the documents required for the ACP and fill it out completely. You can download and fill out documents in 9 different languages, mainly English and Spanish. So, The documents required for the application are the application form and the household worksheet.

How do I enroll in the Affordable Connectivity Program? (ACP)

There are different requirements to be accepted into the Affordable Connectivity Program. If you or any of the members of your household have any of these conditions, you can apply to the ACP program. These conditions are:

-Based on your household income (your income is 200% or less than the Federal Poverty Guidelines)
